How to Apply

If you or someone you know is between the ages of 11-18 and could benefit from orthodontic treatment (braces), but finances keep you from achieving this dream, the Smile for a Lifetime Program may be beneficial for you. To find out if a local Smile for a Lifetime program/chapter is near you please follow the instructions below.

  1. Click here to locate your local chapter and contact them directly.
  2. Look for our logo or a link to their Smile for a Lifetime page on their website. (If you can't find one then call the office directly for more information.)
  3. View the qualifications and if you qualify, fill out the application and submit required paperwork to your local chapter as instructed.
  4. Most chapters will ask for: a) an application, b) letters of recommendation, c) close-up of the child's teeth (Teeth must be clearly visible) and d) Proof of financial status in order to qualify. (Ask your local chapter for specific income eligibility information).

All interested patients must be residents of your local chapters service area in order to apply. The scholarship program is competitive, not all applicants will receive a scholarship. Each application will be reviewed and awarded by a local board of directors. Most boards will meet 2 -4 times a year to screen applicants. Please contact your local chapter in order to find out how often applications are reviewed.
