How to Participate

It is commonly observed and reported that improved appearance can have a  significant impact on a person’s academic, professional and personal success. By establishing a local chapter of the Smile for a Lifetime Foundation in your community, you’ll be helping an individual in need receive a beautiful, healthy smile they can be proud of.

If you are an orthodontist interested in starting a local chapter in your area, please complete and submit the form below. This application allows the board to ensure proper coordination between the orthodontists and chapters.

NOTE: In the application below, in regards to your "proposed" board members, the national board recommends a structure similar to the following: CPA, Banker, Attorney, School Administrator(s), Teachers, Social Workers, Community Advocates (Rotary Officers, Junior League, non-profit staff), Marketing Professional, Media Professional(s) (Print, radio, television), Dental Hygienist, Oral Surgeon or Dentist.

5. Being a part of a research group helping to define the impact of braces on a young person's life.
